 4대 중증 질환 전액 국가부담 대폭 수정키로

표적항암제 약값·검사료 등 필수적인 부분만 혜택 확대

박근혜 대통령 당선인이 공약한 암·뇌·심혈관 질환·희귀난치성 질환 등 '4대 중증 질환 진료비 전액 보장'이 대폭 수정될 전망이다.

여권 관계자는 5일 "2016년까지 단계적으로 4대 중증 질환에 대해 건강보험 혜택을 단계적으로 확대하되 병·의원에서 진료를 받고 내는 본인 부담금은 현행대로 유지하기로 했다"고 말했다. 현재 본인 부담금은 암·뇌질환·심혈관 질환이 건강보험 적용 진료비의 5%, 희귀난치성 질환은 10%다.

이 관계자는 또 "재정 부담이 큰 선택 진료비와 상급 병실료, 간병비는 지금처럼 건강보험 혜택을 주지 않기로 했다"고 말했다.

이에 따라 건강보험 혜택 확대는 그동안 건강보험에 적용되지 않았던 표적 항암제 약값과 검사료·주사료·치료재료비처럼 의학적으로 필수적인 치료 부분에 국한될 것으로 보인다.

당초 박근혜 대통령 당선인은 대선 기간 중 4대 중증 질환 총진료비(기존 전액 본인 부담한 비급여 진료비 모두 포함)를 전액 국가가 책임지겠다며 건보에서 받는 혜택 비율을 75%에서 올해 85%, 내년 90%, 2015년 95%, 2016년 100%로 확대하겠다고 공약했다. 그러나 이 공약대로 할 경우 2014~2017년 4년간 22조원가량이 더 들어 국민이 내는 건강보험료 부담이 커질 것으로 한국보건사회연구원은 추계했다.
